ILCO Authorization for Disclosure Form

Each ILCO Authorization for Disclosure Form is $50 + HST.

The purchase and completion of this form is required for all verification requests.

Please follow the following verification process instructions:

1. Purchase ILCO Authorization for Disclosure Form. Downloadable form will be sent to purchaser via email.
2. Submit completed form for review and approval to general@ilco.on.ca with your full name and name of company requesting the verification. (If you do not have an ILCO member number, please leave the member number field blank.)

Once the request is approved, the requested information will be sent to the requestor and the member/student will be notified that the request is complete.

Important Information:

Please allow for 7-10 business days for processing.

All verification requests will be subject to the member/student’s signature being cross referenced and verified with one on file. Should ILCO not have a signature on file, the member/student will be advised, and they must schedule a time to verify their signature in person at the ILCO office located at 20 Adelaide St E #502, Toronto, ON M5C 2T6.
